    What is Required For SSI Benefits?

    There are required documents you may be asked for in order to apply for SSI. The following is a list of these documents:

    • All contact information for hospitals and doctors you have seen that have documented your disability
    • Proof of citizenship or permanent residence
    • Bank account information for direct deposit
    • All financial records for your assets and the things that you own such as payroll slips, insurance policies, and bank books.
    • Your lease or mortgage slips for where you live
    • Birth certificate
    • Proof of social security number, or the card itself

    How Can I Apply For SSI Benefits?

    To apply for SSI, you must fill out the required paperwork and visit a local office. There is no online application for SSI. The procedures are also different for children and for adults:
